Bilingual Case Management Specialist (Remote Flexible, Spanish Speaking)
Pair TeamCore Responsibilities
Maintain ongoing caseload of individuals using evidence-based approaches to promote engagement and health goals. Conduct periodic telephonic and SMS outreach for timely follow-up with members. Collaborate with care management teams on case reviews and clinical actions. Assist individuals in securing connections to community supports and coordinate care management appointments.
Requirements
1+ years of general work experience, preferably in Case Management. Must be physically located in California. Reliable transportation required for community engagement. A quiet, HIPAA compliant, and internet-connected space is necessary for virtual operations.
